also offer a variety of convenient locations
throughout the bi-state area, staffed with
trained professionals whose experience in the
eyecare field averages 25 years. To take

FREE EYE EXAMINATION
If you use your own personal eye care professional
for your eye exam, add an additional $10 to the

When purchasing a frame and lens package, you may
choose from our selection of designer frames and receive a
discount of:
$75 off on frames valued at $150 or more
$50 off on frames valued at $149 or less
Additional Savings
20 percent DISCOUNT off the regular retail price for
all eye care accessories, including non-prescription
sunglasses, lens add ons and treatments, including
tints, UV Coatings, scratch-resistant coatings, antireflective
coatings and mirror coatings.

Prices include three months
follow-up visits, lenses and care kit. A Service
Agreement may be offered to the consumer at
optometrist’s discretion.
Specialty lenses are available at an additional charge.
How To Use Your Vision Benefits
- Eligibility
To determine eligibility call the UFCW Welfare
Fund Office at 314-835-2700 or toll-free at
1-866-565-2700.
- Eye Examination
Select a UECN provider from the enclosed list, call
to schedule an appointment for an eye exam and
identify yourself as a plan participant of UFCW
Local 655 Welfare Fund.
- Eyeglass Benefits
Bring your prescription in, or we can duplicate
your present prescription - no appointment
necessary. Call individual location for hours.
- No Claim Forms To Complete
All locations will provide the forms and process
the claims.
Package Upgrade
If you choose or are prescribed eyewear not covered
by the Basic Package, vision care benefits will be
deducted from your total cost of the eyewear.
As an added benefit, all SJMECN locations will provide
eyewear not covered by the Basic Package at
specially reduced fees exclusively to plan
participants/dependents of UFCW Welfare Fund.
All pricing is contingent upon UFCW Local 655
Welfare Fund Plan participants and their immediate
families' eligibility and the assignment of fund
benefits to UECN Providers.
