If you use your personal eye care professional for your eye exam, you may still purchase eye glasses at a member location by bringing in your prescription. Be sure to present your ID card to the provider at the time you receive care or purchase eye wear. The products and services described relative to eyeware are neither offered or guaranteed under the PremierPlus contract with the Medicare program, but are made available to all enrollees who are members of PremierPlus. These products and services are not subject to the Medicare appeals process. Any disputes regarding these products and services may be subject to the PremierPlus grievance process.
