Your have the following options:
- Premier Basic
- Premier Premium
- No Coverage
Premier Basic Option
The Basic Option pays a fixed amount for eye exams and services. Any charges over the limit are not covered. Since you receive a lower lever of coverage, you pay a lower premium.
Premier Premium Option
This option will pay 80% of charges for eye exams, lenses and frames up to a $350 annual maximum per person. Since you receive a higher level of coverage, you pay a higher premium.
